Important Documents

Before each session I will email you a link to a portal that supplies you with the necessary forms.  These forms include informed consent, privacy practices, practice policies, demographics, a credit card form for no shows or late cancellations, and intake questions. They do not take long to fill out and are all completed online with secure and HIPAA compliant software.  Feel free to contact me if you have any questions about my forms.
